If you are billing for multiple providers and they each have different pricing for the same procedure code you may enter the same code you used in the provider file so the correct pricing will default when entering claims (may leave blank). 

CPT Code 
Enter the CPT/HCPCS code for the procedure to be added to the file. The code must be valid, (i.e., exactly as stated in the CPT manual or the HCPCS listing published by Medicare Part B). 

Modifier 1 to 4 
If applicable, enter modifiers that will pertain to the CPT Code. 

Mnemonic Code 
Enter any code or word that would easily identify this CPT/HCPCS procedure. Example: If you put in procedure code 99202 and gave it a mnemonic of ov, when typing in the service line you could enter ov and it would pull in 99202. You may leave this blank. 

Standard Charge 
Enter the standard charge (will be the Medicare default charge) for this procedure. 

Cat of Svc 
Type in the category of service issued by Illinois Department of Public Aid (IDPA) if applicable. 

Description 
Enter the description of this code. You may leave this blank. 

Procedure # 
You do not need to fill this section out UNLESS your standard charge varies per payer. Enter the code used for this procedure on the appropriate line for the insurance type.
